This may be due to an incorrect needle procedure, needle defect, tube blockage, problem with the apheresis machine, or improper action by the phlebotomist. Definition: If this is donation has an intended recipient (autologous, directed, dedicated, designated), the intended recipient’s name.

Definition: The Staff member conducting the final review and reconciliation of all documentation created during the collection process.

Definition: The number of samples collected during the donation which will be used for subsequent testing.
